From 1 November 2025, Intrinsic Health will deliver allied health services under the Australian Government’s new Support at Home Program, replacing Home Care Packages (HCP) and Short-Term Restorative Care (STRC).

Support at Home provides a simpler and more flexible way for older Australians to access the help they need to live independently and confidently at home.

Support at Home is designed to make aged care more straightforward and responsive. It provides older Australians with access to services that help them stay at home for longer and maintain their health, mobility, and independence.

The program funds a range of supports — including allied health, nursing, assistive technology, and home modifications — delivered by registered providers.

At Intrinsic Health, our clinicians work closely with clients, families, and care coordinators to tailor support to individual goals.

We provide the following allied health services under Support at Home:

  • Occupational Therapy – home safety, equipment prescription, and home modifications
  • Physiotherapy – strength, balance, mobility, and falls prevention
  • Speech Pathology – communication, swallowing, and mealtime management

All services are available in-home, in-centre, or via telehealth, depending on your needs and location.

Register your details and request an aged care assessment.


2. Receive your assessment outcome confirming eligibility and funding level.


3. Choose Intrinsic Health as your provider.
We will work with you to design a plan tailored to your goals, routines, and preferred way of living.


If you are already receiving a Home Care Package, you do not need to reapply — your services will automatically transition to the new Support at Home Program.

Who/what does the At Home service cater to?
<p class="rteBlock">Tailored for HCP, STRC, CHSP, DVA clients and NDIS participants, the service is based on an assessment of the client and their needs within a home enviroment. Our therapists collaborate with families and coordinators to support clients of all ages, focusing on skill acquisition, rehabilitation, restorative care, and participation in meaningful activities.</p>
What does ‘home modification’ by an OT entail?
<p class="rteBlock">Home modification by an occupational therapist (OT) involves assessing and adapting living environments to enhance safety, accessibility, and independence. This process can include minor adjustments like installing grab rails, threshold ramps, and  portable external ramps. More complex renovations include full bathroom remodels and custom built access solutions. The OT’s role is to ensure that these modifications meet the specific needs of the client, taking into account their current and future requirements for a safe and functional home. </p>
Can I customise my At Home services?

Is there a minimum commitment for At Home services?
<p class="rteBlock">There is no mimimum commitment. We offer flexible service arrangements to suit your needs, whether you require short-term assistance or long-term support.</p>
What is the Support at Home Program?
<p class="rteBlock">Support at Home is the new government-funded aged care program launching on 1 November 2025. It replaces the Home Care Package and Short-Term Restorative Care programs, simplifying access to in-home support for older Australians.</p>
Do I need to do anything if I already receive a Home Care Package?
<p class="rteBlock">No — your current services will automatically transition to Support at Home. You do not need to reapply or change providers, and your level of support will continue without interruption.</p>
What is changing under Support at Home?
